The Income-Related Monthly Adjustment Amount (IRMAA) affects about 8% of Medicare beneficiaries. For 2026, thresholds are $218,000 for joint filers and $109,000 for singles. Events like a spouse’s death can halve these thresholds, but filing SSA-44 triggers an immediate recalculation.

Voluntary actions such as Roth conversions or capital gains disqualify filers from SSA-44 relief, locking in IRMAA surcharges. Two couples retiring in early 2025 saw premiums differ by $81 monthly solely due to one filing the form.
