Insmed (INSM) increased its peak sales forecast to more than $14 billion, a 75% jump from prior estimates, citing strong performance from its bronchiectasis drug BRINSUPRI and pipeline growth. The company reported $1.1 billion in trailing twelve-month revenue, nearly tripling from $0.4 billion a year earlier.

BRINSUPRI generated $309.2 million in Q2 2026, up 49% sequentially, while ARIKAYCE contributed $116.3 million, an 8% year-over-year increase. Full-year 2026 guidance for BRINSUPRI was raised to $1.25 billion-$1.4 billion, from over $1 billion. Despite revenue growth, the company’s operating margin remains deeply negative at -71%, compared to the S&P 500’s 18.5%.

Over $6 billion of the peak revenue estimate hinges on drugs still in Phase III trials, underscoring reliance on pipeline execution. Combined R&D and SG&A spending rose 38% year over year to support launches and development.
